Liability Release and Assumption of Risk Agreement for Nitrox Diving

I, , hereby affirm that I wish to participate in scuba diving with Nitrox (enriched air), and that I am aware that these activities have inherent risks and may result in serious injury, death, or property damage. I understand that these activities may require significant travel by land or sea, require strenuous physical activity, swimming, and carry both specific and general inherent risks, including but not limited to the use of Nitrox, which carries additional risks such as oxygen toxicity, decompression sickness, and other potential hazards specific to the use of Nitrox

I understand and accept the risks associated with the use of Nitrox, including the need for appropriate training, proper equipment, and careful monitoring of dive profiles to mitigate these risks. I acknowledge that any failure to follow the required procedures and guidelines when diving with Nitrox can result in serious injury or death.

In consideration of being allowed to participate in such activities, I hereby personally assume all risks, both foreseen and unforeseen, that may arise or befall me as a result of undertaking this activity, including those associated with the use of Nitrox.

I agree to unconditionally release and discharge all liability to any and all duly certified, registered in active status, NovoScuba instructors, assistants, guides, or leaders that may take part in the supervision of my activity.

Daarnaast ontsla ik onvoorwaardelijk alle aansprakelijkheid van elk duikcentrum/winkel/resort dat naar behoren is geregistreerd als NovoScuba-lid, waar ik deelneem aan enige activiteit die onderdeel uitmaakt van mijn NovoScuba-ervaring.

NovoScuba.com (including their affiliates, agents, subsidiaries, and employees) are unconditionally released from any responsibility and liability, in any way, for any injury, death, or any other damages to me or my property, arising from or related to my participation in this Activity, including but not limited to, any risks associated with the use of Nitrox. I understand and agree that this release applies not only to me, but to my family, estate, or heirs, as well as any party related in any way to me. I understand that this release applies for participation in this activity, including travel to and from the activity, and due to any negligence of any party. I also understand that I am waiving my rights to any legal claim or lawsuit arising from my participation in this Activity.

Ik heb de NovoScuba DiveSmart-overeenkomst, including specific guidelines regarding the use of Nitrox, and I have read and understood these guidelines. I understand that I am taking full responsibility for myself during participation in the Activity, and I confirm that I meet the necessary training and certification requirements to safely engage in Nitrox diving.

Ik ben meerderjarig en bekwaam om deze overeenkomst te ondertekenen. Indien wettelijk vereist (vanwege mijn leeftijd of om een andere reden), heb ik de schriftelijke toestemming van een ouder of wettelijke voogd verkregen, en zij hebben ermee ingestemd zich aan de voorwaarden van dit contract te houden.
